Healthcare: Patient 360 & Record Linkage
Patient 360: one trusted identity across every system.
Resolve the same patient across EHR, claims, lab, pharmacy, and device data into a canonical patient graph: the foundation for care coordination, risk stratification, and every downstream clinical AI.
The short answer
What is Patient 360 and record linkage?
Patient 360 is a single, unified view of a patient assembled from every source system: EHR, claims, lab, pharmacy, and devices. Getting there requires record linkage (also called patient matching or entity resolution): deciding that records with slightly different names, addresses, or IDs actually belong to the same person. A graph based master patient index (EMPI) resolves those records into one canonical patient node with the match evidence attached, so every downstream system (care coordination, risk stratification, clinical AI) reads from the same trusted identity.
What we build
Patient 360 & Record Linkage, engineered on your data.
Graph entity resolution
Resolve the same patient across systems using probabilistic and graph based matching: stronger on messy demographic data than deterministic rules alone.
Auditable match provenance
Every match carries the evidence and confidence that produced it, so compliance, appeals, and merges are fully explainable.
Cross domain unification
One patient ID reconciled across EHR, claims, lab, pharmacy, and device data: clinical, administrative, and financial in one record.
Care coordination foundation
A canonical patient graph is the substrate for care gap detection, transitions of care, and cross provider coordination.
Risk stratification
Cohort and patient level risk scoring that reads a complete, de duplicated history instead of a fragmented one.
Trusted input for AI
Every clinical AI application is only as reliable as its patient identity: Patient 360 is the grounding layer beneath GraphRAG and decision support.
FAQ
Patient 360 & Record Linkage: frequently asked questions.
What is a master patient index (MPI / EMPI)?
A master patient index is the system of record for patient identity: it assigns and maintains one enterprise ID per real person and links every source system record to it. An EMPI (enterprise MPI) does this across multiple facilities or systems. On a graph, the EMPI becomes a living network of identity evidence rather than a static table.
Why is graph based record linkage better than deterministic matching?
Deterministic matching requires fields to agree exactly, so it misses patients whenever a name, date, or address differs: and healthcare data is famously messy. Graph based linkage weighs many signals together, uses relationships (shared address, provider, insurance) as evidence, and keeps the match auditable, which lifts match rates without silently merging different people.
How does Patient 360 support clinical AI?
Every clinical AI answer is only as trustworthy as the patient record behind it. If the patient's data is split across three unlinked records, the AI reasons on a partial picture. Patient 360 gives GraphRAG and decision support one complete, de duplicated, provenance tracked history to ground answers in.
