What is Patient 360 and record linkage?

Patient 360 is a single, unified view of a patient assembled from every source system: EHR, claims, lab, pharmacy, and devices. Getting there requires record linkage (also called patient matching or entity resolution): deciding that records with slightly different names, addresses, or IDs actually belong to the same person. A graph based master patient index (EMPI) resolves those records into one canonical patient node with the match evidence attached, so every downstream system (care coordination, risk stratification, clinical AI) reads from the same trusted identity.

What is a master patient index (MPI / EMPI)?

A master patient index is the system of record for patient identity: it assigns and maintains one enterprise ID per real person and links every source system record to it. An EMPI (enterprise MPI) does this across multiple facilities or systems. On a graph, the EMPI becomes a living network of identity evidence rather than a static table.

Why is graph based record linkage better than deterministic matching?

Deterministic matching requires fields to agree exactly, so it misses patients whenever a name, date, or address differs: and healthcare data is famously messy. Graph based linkage weighs many signals together, uses relationships (shared address, provider, insurance) as evidence, and keeps the match auditable, which lifts match rates without silently merging different people.

How does Patient 360 support clinical AI?
